"In inter-war Sweden, the psychiatric diagnosis of psychopathy served as a controversial but powerful tool for the management of people who failed to live up to contemporary civic ideals. The diagnosis was based on theories of a broad spectrum of biologically-based disorders in the borderland between normality and actual mental illness. The disturbances were assumed to manifest themselves as, for example, homosexuality, hysterical attacks, emotional coldness, mythomania, or restlessness.Another controversial diagnosis was paranoia querulans or querulous paranoia, a disease that was linked to the emergence of the modern state and its rule of law, and thought to manifest itself mainly in hyperbolic complaints against supposed wrongdoings. Thus, the disorder implicated an excessive use of one's civil rights. A particular dilemma related to this disease was that the kind of behaviour that had usually led to the diagnosis and subsequent psychiatric confinement, namely a ardent manner of writing letters to public authorities, was precisely what was required for discharge.In De samhällsbesvärliga (The Social Troublemakers), historian of science and ideas Annika Berg examines how patients described as psychopaths or querulants could negotiate for discharge with doctors and authorities in 1930s and 40s Sweden. This was a period in time when the system of psychiatric care in Sweden was greatly expanded in answer to a perceived shortage, but was also transformed into a somewhat more open apparatus with possibilities to apply for preliminary discharge and other forms of outpatient solutions. It was also a time when psychiatry was under attack from different quarters, and psychiatrists were accused of using flexible diagnoses such as psychopathy to confine people wrongly. Against this backdrop, how did the management of psychopaths and querulants fit with contemporary ideals of citizenship and democracy? How did the patients view themselves? And how were negotiations in individual cases affected by notions of, for example, class, gender and sexuality?"

Few afflictions are as frightening or as heartbreaking as mental illness. It may be a topic that many would prefer to sweep under the rug, but it is a fact of life that we as a society can and must face. We have come a long way over the past few decades in our understanding of mental illness and its potential treatments. Yet, tragically, many across the country who struggle with serious mental illness are unable to find effective, quality medical treatment. As a federal commission on mental health concluded, the system of care is in shambles. But why? And how do we fix it?Timothy A. The International Workshop on Models and Analysis of Vocal Emissions for Biomedical Applications (MAVEBA) came into being in 1999 from the particularly felt need of sharing know-how, objectives and results between areas that until then seemed quite distinct such as bioengineering, medicine and singing. MAVEBA deals with all aspects concerning the study of the human voice with applications ranging from the neonate to the adult and elderly. Over the years the initial issues have grown and spread also in other aspects of research such as occupational voice disorders, neurology, rehabilitation, image and video analysis. MAVEBA takes place every two years always in Firenze, Italy.

This book of Proceedings collects the papers presented at the 3rd International Workshop on Models and Analysis of Vocal Emissions for Biomedical Applications, MAVEBA 2003, held 10-12 December 2003, Firenze, Italy. The workshop is organised every two years, and aims to stimulate contacts between specialists active in research and industrial developments, in the area of voice analysis for biomedical applications. The scope of the Workshop includes all aspects of voice modelling and analysis, ranging from fundamental research to all kinds of biomedical applications and related established and advanced technologies.

Because Reserve Component (RC) members have been increasingly used in an operational capacity, among the policy issues being addressed by the 11th Quadrennial Review of Military Compensation (QRMC) is compensation and benefits for the National Guard and Reserve. As part of the review, RAND was asked to analyze healthcare coverage and disability benefits for RC members, including participation in the TRICARE Reserve Select (TRS) program, the potential effects of national health reform on coverage rates, and disability evaluation outcomes for RC members. This report summarizes the results of RAND's analysis. The author finds that 30 percent of RC members lack health insurance to cover care for non-service-related conditions. The TRS program offers the option of purchasing health insurance through the military on terms that are superior to typical employer benefits. Although program participation has increased, it remains low and TRS does not appear to be effectively targeting those most likely to be uninsured. TRS premiums are also lower than the premiums for the new options that will be available under health reform and the same as the penalty for not being insured. So health reform is likely to increase TRS enrollment. Finally, previously deployed RC members are referred to the Disability Evaluation System at a much lower rate than Active Component (AC) members, even for deployment-related conditions, but those who are referred receive dispositions (and thus benefits) similar to those for AC members. These findings suggest that the Department of Defense may want to consider ways to better coordinate TRS with other insurance options that will be available to RC members and that the identification of RC members who experience health consequences from deployment leading to disability merits further investigation.
